DEBIT AUTHORIZATION CONFIRMATION LETTER

Phiz App LTDA ("Phiz"), in compliance with Brazilian legislation, issues this Debit Authorization Confirmation Letter ("Letter") and authorizes the automatic debit service ("Service").

READ CAREFULLY before agreeing. This Letter contains important clauses about your rights and obligations, including liability disclaimers (highlighted in bold). By clicking "I Agree", you expressly accept all conditions below and authorize Phiz to perform automatic debits as described.

1. Automatic Debit Authorization
You authorize the Merchant (Phiz's commercial partner) to send Debit Instructions to Phiz, which may debit amounts directly from:

  1. Bank account linked to your account
  2. Other registered payment methods ("Debit Channels")

No additional password, SMS, or other authentication will be required for subsequent debits.

2. Merchant Information and Transparency
  1. Phiz will display Merchant details (name, CNPJ, amount, frequency, and purpose) before confirmation
  2. Detailed statements will be available via app, e-mail, or SMS

3. Responsibilities
  1. You are responsible for maintaining sufficient balance in Debit Channels
  2. In case of insufficient balance, Phiz may try other registered Debit Channels, conforming to this authorization
  3. Phiz is not responsible for disputes between you and the Merchant (e.g., undue charges). These cases must be resolved directly with the Merchant

4. Cancellation and Changes
  1. You can revoke the authorization at any time through the app or customer service channel, subject to pending debit settlement
